Our Transition Age Youth program is designed to help youth between the ages of 16 and 22 who are struggling with significant emotional or behavioral issues as well as substance use issues so that they can make a successful entry into the adult world. This is achieved through clinical assessment and treatment, employment supports, case management, and assistance in getting access to physical health care services.
Youth served in this program may also work with our outpatient clinical program, our Youth in Transition Program (in Springfield only), and our JOBS program. These programs can help eligible youth prepare for the workplace by assisting youth in preparing their resume, coaching them in how to prepare for and act in a job interview, and can teach core skills in how to succeed in a paid work environment. Case management services are also available to assist youth with education, housing, independent living skills, and connecting to other resources and supports in the community.

Being able to have appointments through Zoom has been a huge help. Everyone is very reachable by either phone or email…I feel like I can reach out to either our case manager or counselor at any time, which helps with making me feel very supported.
